Life Planning Worksheet
This is a resource that my husband and I developed when we started caring for my elderly mother. It helped us understand where we stood in terms of her ability to live independently and assess the level of care she needed at each level.
We found that these levels were NOT STATIC. She could move one column to the right for a few weeks, “improve”, and move back to the previous level.
We do understand that, ULTIMATELY, we are all MOVING TO THE RIGHT … the question is just WHERE we are at any given point in time.
